Important Notes

This Nomination Form is to be completed by the takaful participant who has attained the age of eighteen (18) years old.
Participant may appoint nominee as Executor or Beneficiary.
Nomination of Executor or Beneficiary is allowed only if the participant is the Person Covered.
The Executor is a trustee who is responsible to distribute the takaful benefits, subject ot the conditions below:

For Muslim Participant, the Executor has to distribute the takaful benefit(s) according to the Islamic Law and Law; or
For Non-Muslim Participant, the Executor has to distribute the takaful benefit(s) according to Chapter 11, Probate and Administration Act 1956, Letter of Administration or Grant of Probate.

The Beneficiary is a recipient of the takaful benefits on the basis of Conditional Hibah (Gift). Conditional Hibah has the effect of transferring ownership of the takaful benefits payable to the Beneficiary upon the death of the participant and shall not form part of the estate of the participant or be subject to his/her debts.
The latest submission and endorsement of any nomination by the Company shall supersede any previous nomination made.

Notes

Executor or Beneficiary must submit a copy of Identification Card or Passport
If the participant appoints two (2) Beneficiaries under the Conditional Hibah and one of the nominated Beneficiaries passes away before the participant, the share of the deceased Beneficiary will be paid to the participant’s heirs*, upon the death of the participant, unless the participant has made a new nomination replacing the deceased Beneficiary.
If the participant chooses Executor as nominee, it is advisable that the participant appoints two (2) Executors. If the first Executor appointed by the participant has passed away, the second Executor will act as a trustee who is responsible to distribute the takaful benefits to the participant’s heir.
If a nominee dies after the participant, and the Company has not yet paid the takaful benefits to the nominee, the Company will pay the takaful benefits as follows:
- if the nominee is an Executor, the takaful benefits will be paid to the participant’s heirs*; or
- if the nominee is a Beneficiary, the takaful benefits will be paid to the heirs* of the deceased nominee
If the participant has not chosen a nominee, or if the appointed nominee passes away and the participant has not named a new nominee, the takaful benefits will be paid to the participant’s heirs*.

* Takaful benefits that will be paid to the heirs shall be through executor that is appointed by the Court.
